Emergency Patient Response Role Chart | Health Managers · Supervisors · HR · Security

When rewriting the emergency situation manual, there are too many columns ending with "handled by the health manager." This Excel spreadsheet covers 21 tasks, from discovery to post-incident reporting, divided among four roles: health manager, supervisor, HR, and security. It is designed to automatically display empty columns by shift, statutory reporting targets and deadlines, and elapsed time by time of occurrence.

What you can do with this material

This is a file that assigns who calls 119 and who goes to the hospital by shift. First, please fill out the two sheets: 'Role Chart' and 'Assignments by Work Group'.

  • Assigning responsibility for each task: The primary person in charge is indicated for 21 tasks, including treatment, reporting, accompanying, and contacting family. A warning is displayed if there is no primary person in charge or if there are two or more.
  • Finding empty seats at night and on weekends: If you enter the name of the person in charge for each shift, the blank space will be displayed as 'blank'. Please fill in the names of the people responsible for treatment, reporting, and accompaniment during times when the health manager is unavailable.
  • Preparing the message to convey to the hospital: Please print the 'Companion Card' and place it in the first-aid kit or security office. It contains information to convey to medical personnel, such as the time of incident, on-site treatment, and company contact details.
  • Checking Records and Reporting Deadlines After an Accident: If you enter the times of discovery, reporting, and treatment, the elapsed time is calculated. In 'Reporting Determination,' you can check whether the incident is subject to reporting and the submission deadline by entering the date of occurrence and damage status.

※ The default role values are examples to be modified to suit your company. The reporting judgment is a verification tool based on input values, and the final judgment, such as business relevance, must be made separately.

📋 Fill out the role sheet based on the 'one main person in charge' principle.

For each task, select one of ● (Primary), ○ (Support), or a blank space in the four role boxes. If a task has no ● or has two or more, a warning will appear in the confirmation box on the right. This is because tasks marked as 'Everyone does it' are likely to end up being done by no one in actual situations. Below are some of the default values for the 21 tasks.

step work Health manager Supervisor HR security
Discovery/Report Call 119 and maintain the call. ○ ●   ○
Treatment Check consciousness and breathing, chest compressions, use AED ● ○   ○
site Stop work, clear surrounding personnel ○ ●
site Ambulance Gate Information, Access Control   ○   ●
transfer Designate a companion, hand over the companion card ○ ●
liaison Contact family, share hospital information   ○ ●
report Determination of legal reporting targets, preparation of reporting ● ○ ○
recovery Field Staff EAP Counseling Guide ●   ○

The default values are examples. Businesses without security personnel may simply change the name of the Security column to 'Facility Manager'. The aggregation line below the role table shows the number of ●s for each role, so please also check to ensure that primary responsibilities are not concentrated in a single role.

🌙 Find the times when the Health Manager is unavailable using the shift assignment chart.

If the role chart is about 'what and who does,' the shift assignment chart is about 'who is actually present at that time.' For each of the four shifts—day, night, weekend, and holiday—enter the names of the personnel responsible for treatment, calling 119, gate guidance, escorting, contacting HR, and reporting decisions. If a name is missing, 'Blank' is displayed, and for shifts where the treatment 담당 is not a health manager, 'First Aid Training Confirmed' is displayed. If you fill in the training completion date column, it changes to 'Review for Retraining' for those who have passed two years. Since the two-year period is an internal standard example rather than a statutory cycle, it can be changed on the sheet.

🧾 Instantly check statutory reporting targets and deadlines with the Reporting Decision Sheet.

If you simply enter the date of occurrence, work-related status (Yes/No/Under Verification), number of fatalities, number of injured persons requiring medical treatment for more than 3 months, number of simultaneous injuries or illnesses, and the number of days of leave required, Excel will display whether the incident constitutes a serious accident under Article 3 of the Enforcement Rules of the Occupational Safety and Health Act, whether it is subject to the Industrial Accident Investigation Report under Article 73 and the submission deadline (1 month from the date of occurrence), and the record retention period under Article 164 of the Act (3 years). If you set the work-related status to 'Under Verification,' it calculates the deadline in advance and displays it as 'Decision Needed.' If you enter three cases as a hypothetical example, the output looks like this.

Virtual example input serious accidents Survey form Submission deadline
Fractured wrist after falling while loading Business-related example, 21-day closure Or not Target Date of occurrence + 1 month
Fainting in the office due to a personal illness Business-related No, Closed for 2 days Or not Not a target Internal records only
Transferred due to chest pain during night work Checking business-related matters, closed for 5 days Judgment needed Judgment needed Preparation based on date of occurrence + 1 month

This judgment is an auxiliary function that compares input values against clause requirements. It does not determine the work-relatedness itself, so for cases marked 'Under Verification,' the Safety and Health and Labor Relations managers should reach a conclusion together.

⏱️ The incident record and companion card are written immediately on the same day.

In the incident record sheet, enter the times of discovery, calling 119, initiation of treatment, arrival of AED, arrival of ambulance crew, departure from scene, arrival at hospital, contact with family, and internal reporting in sequence. The elapsed minutes from the time of discovery are automatically calculated, and if you enter a time earlier than the previous step, 'Time Verification' will appear. Since memories fade after just one day, it is most accurate to fill this out immediately after a mock drill or actual situation ends. This record can be used as the cause record under Article 57 of the Act and as semi-annual inspection data under Article 4, Paragraph 8 of the Enforcement Decree of the Serious Accidents Punishment Act.

The companion card is a single-page printable sheet. It is divided into sections for information to convey to medical personnel (time of collapse, immediate work, on-site treatment and time, substances handled, company contact details) and information not to do (mentioning pre-existing conditions without consent, assuming the cause of death), so you can keep it in a first-aid kit or the security office.

❓ FAQ

Q1. Can workplaces with fewer than 50 employees that do not have a health manager also use this?

You can use it. Change the Health Manager column to 'First Aid Officer' and fill in the training completion date for the treatment officer in the shift assignment table. The clause comparison on the Report Judgment Sheet is the same regardless of the size of the workplace.

Q2. Should AED inspections be performed monthly?

In accordance with Article 47-2 of the Emergency Medical Services Act, workplaces subject to mandatory requirements (those requiring the appointment of a health manager + 300 or more regular employees) must conduct inspections at least once a month and report the results. If a workplace is not subject to this obligation, the company may determine the frequency, but the default value in Excel has been set to one month to maintain the same standard.

Q3. Do I need to submit the role chart to the Industrial Safety and Health Committee?

Although it is not a procedure prescribed by law, if you intend to include the division of roles in the safety and health management regulations or emergency response manuals, it is advisable to undergo committee deliberation or internal approval. This is because the duties of a health manager include 'duties stipulated in the safety and health management regulations and employment rules' (Article 22, Paragraph 1, Subparagraph 1 of the Enforcement Decree).

📚 Source and Verification Criteria

Legal Verification Date: October 28, 2026. This role table and Excel file are practical forms and not statutory forms. The default values for roles, retraining (2 years), and inspection cycles are examples based on internal standards, and the cases in 'Hypothetical Examples' are assumptions for illustrative purposes. The Reporting Judgment Sheet is a function designed to assist in comparing requirements of the provisions and does not replace legal judgments on individual cases, such as work-relatedness.

👉 To ensure employee recovery following emergencies

The last column of the role chart is the recovery of employees who were on-site. Nudge EAP supports the operation of the in-house psychological counseling center and connects employees with individual counseling during accidents and crises, and provides the company with only aggregated reports that do not identify individuals.
