What questions would be good to use in an EAP satisfaction survey to assess service quality without asking about the counseling content?

Hello, I am the person in charge of employee benefits at the HR team of an IT company.
It has been about two years since we implemented the EAP, and we plan to check employee satisfaction while reviewing contract renewals at the end of the year.
The monthly report provided by the company does include satisfaction scores, but perhaps because they are received immediately after the consultation, almost all of them came out high, so it felt a bit awkward to rely solely on that for the report.
So, I'm thinking of running a short separate survey at the company level, but now that I'm actually trying to formulate the questions, I'm not sure how far I should go with them.
I included something like "What concerns did you seek counseling for?" in the initial draft, but my team leader said it wasn't something the company should be asking, so I removed it.
I intend to exclude the consultation details and focus only on service aspects, such as whether the booking was convenient and if I could receive the appointment at my desired time, but I am not sure if I can judge the quality based solely on that.
Also, since the company doesn't know who used it, I think we should send it to all employees, but I expect the responses will get mixed up because there are likely more people who haven't used it.
I am also wondering if it is permissible to include questions that only those with usage experience can answer, and if things like department or job title should not be included.
I would appreciate it if you could share what questions other companies use for EAP satisfaction surveys and whether they conduct them separately from vendor surveys.
